Certification Requirements

  • License Practical/Vocational Nurse with current licensure to practice professional nursing in the State of Wisconsin.
  • Graduate of an accredited practical nurse or vocational nursing program.
  • Prefer a minimum of one year experience in a home health organization.
  • Current American Heart Association CPR/BLS for Healthcare Providers.
  • Promote and maintain a team-oriented environment.
  • Shows ability to organize and prioritize workload independently.
  • Must be able to follow direction and accurately report to the supervisory staff any changes in patient’s condition.
  • This position requires frequent travel between patient homes; must be a license driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.

Job Duties

  • Provide skilled nursing treatments to patients according to treatment plans developed by a Registered Nurse.
  • Assist the Registered Nurse on evaluations and complex treatment procedures.
  • Observe, record, and report to the Registered Nurse the patient conditions reactions and responses related to treatment.
  • Maintain daily records of treatments and document progress notes on patients as directed by the Registered Nurse.
  • Assist the Registered Nurse in consulting with family members, physicians and other health care professionals as appropriate (Coordinate referrals to additional disciplines (HHA, OT, PT, ST)
  • Assist the Registered Nurse with in-service programs and other meetings as assigned by the Registered Nurse.
  • Access continuing education opportunities appropriate to discipline and responsibilities in order to maintain clinical competency.
  • Demonstrates competence and ability to provide care including but not limited to wound and catheter care as well as medication assessment, patient education for disease management, and prevention of rehospitalization.
  • Attend and participate in staff meetings and other meetings as requested.
  • This position must continually meet the productivity expectations set for by PRN Home Health and Therapy. PRN has the right to adjust each individual employee’s productivity objective independently based upon said employee’s specific position responsibilities.
  • Act in compliance with PRN Therapy Management Policies and Procedures, State, Federal and Professional Organization regulatory standards and guidelines.
  • Participates in ongoing evaluation of the home environment for hazards or barriers to promote more independent living as delegated by the Registered Nurse.
  • Perform other duties as assigned.
