We’re looking for a part-time LPN to join our growing team! This position would be 5-15 hours a week and requires frequent travel between patient homes to provide Bone Density Scanning with a portable scanner. Mileage reimbursement and travel time is paid. Driving would be throughout Eastern Wisconsin, but primarily in the Oshkosh/Fond du Lac areas. You may also perform additional preventative screening tests such as home safety and wellness health assessments to the patients.

Certification Requirements

  • Registered Nurse or Licensed Practical Nurse with current licensure to practice professional nursing in the State of Wisconsin.
  • Minimum of one year experience in a home health organization.
  • CPR/BLS (American Heart Association) upon hire and appropriate renewal.

Job Duties

  • Provide mobile Bone Density Scans to Network members in their home and assisted or independent living facilities.
  • Conduct member home and fall safety assessments, as needed and report to Network contact any concerning patient conditions.
  • Accurately explain the Bone Density Scanning process, hand out educational information as provided and educate appropriately Network members.
  • Consult with family members, physicians and other health care professionals as appropriate.
  • Maintain daily time record of Network member communications, scans, assessments, travel and documentation.
  • Following proper infection control and bag technique, ensure cleanliness and functionality of all scanning equipment.
  • Access continuing education opportunities appropriate to job function and responsibilities in order to maintain clinical competency.
  • Attend and participate in staff meetings and other meetings as requested.
  • Perform other duties as assigned.
