Certification Requirements

  • Graduate of an accredited Occupational Therapy curriculum.
  • Current Occupational Therapy license/certification in Wisconsin.
  • Current American Heart Association CPR Certification.
  • Experience or interest in lymphedema treatment is a bonus.
  • Recent graduates are welcome.

Job Duties

  • Respond to and initiate patient referrals.
  • Administer standardized and non-standardized assessments, analyze data, develop short/long term goals and make recommendations.
  • Maintain daily records of treatments and complete progress notes in compliance with PRN Home Health & Therapy procedures.
  • Record patient progress in the patient record a minimum of every ten (10) visits.
  • Review available patient information and evaluate the impact of this information on the assessment and treatment process.
  • Develop and implement an individualized treatment plan based on patient needs and containing achievable functional goals.
  • Instruct Assistants in treatment procedures to be utilized and provide supervision while such staff is working with patients.
  • Act in compliance with PRN Home Health & Therapy Management Policies and Procedures, State, Federal and Professional Organization regulatory standards and guidelines.
  • Verify all PRN Home Health and Therapy required vaccinations are up to date and reported to the corporate office.
