The Occupational Therapist evaluates and treats patients in accordance with orders from a physician through use of therapeutic procedures and assistive devices intended to maximize functional potential in all areas of a patient’s life. The Occupational Therapist supervises COTAs who provide OT treatments delegated to them for patients under the Occupational Therapist’s care. The Occupational Therapist participates as a member of the interdisciplinary team providing support and information within the area of Occupational Therapy.

Certification Requirements
- Graduate of an accredited Occupational Therapy curriculum
- Current license/certification in Wisconsin
- Current American Heart Association CPR Certification
- Current registration with the National Board for Certification of Occupational Therapy
- Prefer active membership in the American Occupational Therapy Association and the Wisconsin Occupational Therapy Association
Job Duties
- Respond to and initiate patient referrals
- Administer standardized and non-standardized assessments, analyze data, develop short/long term goals and make recommendations
- Select media and activities specifically related to individual patient needs
- Monitor the patient’s conditions, reactions and responses to treatment and implement necessary revisions to the therapy plan of care as appropriate
- Maintain daily records of treatments and complete progress notes in compliance with PRN HOME HEALTH AND THERAPY procedures
- Record patient progress in the patient record a minimum of every ten (10) days
- Provide consultations with family members, physicians and other health care professionals as appropriate
