About the Position:

This On-Call Registered Nurse position is mainly a work from home position in Madison, WI. When on-call our RN must be within 45 miles of the company’s service area. Our on-call team will mostly provide telephone triage for patients and participate in occasional in person visits. Applicant must have a strong work ethic, great communication skills, and a desire to help people and be able to work independently.

Certification Requirements

  • Registered nurse with current licensure to practice professional nursing in the State of Wisconsin.
  • Minimum of one year experience in the home health field.
  • Current American Heart Association CPR/BLS for Healthcare Providers.
  • Strong will to stay organized and prioritizes workload independently.
  • Excellent oral and written communication skills.
  • Must be able to follow direction and accurately report to the supervisory staff any changes in patient’s condition.

Job Duties

  • Answer all phone calls on behalf of the company, and follow up according to company policy and procedures.
  • The hours on-call include: 5 p.m. – 8 a.m. Monday through Friday. Every other weekend from 5 p.m. Friday evening through 8 a.m. Monday morning and provide a minimum of 8-12 hours of hands-on patient care per month during regular business hours.
  • Assume primary responsibility for PRN Home Health & Therapy total census during assigned shift.
  • Provide skilled nursing, intervention, and supportive care to patients in the home setting when needed.
  • Maintain regular communication with company staff and the patient’s referring physician and/or case manager concerning patient/family care needs and response to interventions.
  • Maintain up-to-date patient records so that problems, goals and interventions are accurately and clearly stated and changes are reflected as they occur.
  • Complete documentation within guidelines and to meet regulatory requirements.
  • Submit completed documentation within one business day after on-call services are provided.
  • Accept referrals for new patients in accordance with the company’s policies and procedures.
  • Communicate regularly with company nursing staff to review problems or unique issues from on-call hours.
  • Participate in the company’s orientation and in-service training programs for professional staff.
