About the Job: We’re looking for a Part-Time Physical Therapist to see patients in Green Bay. This position would work in an outpatient setting. Opportunity to grow to full-time if PT wants to work home health hours too.

We’re Looking for a Physical Therapist (PT) Who: 

  • Wants to provide high quality and the best patient care around, primarily to patients in an outpatient setting.
  • Loves working independently, but wants to have a team who is always there to support them when needed.
  • Is a great communicator, both written and orally.

Certification Requirements

  • Prefer a minimum of one year of clinical experience as a Physical Therapist.
  • Graduate of an physical therapist curriculum accredited by the American Physical Therapy Association.
  • Passed the National Physical Therapy Board exam.
  • Current Physical Therapist license in the State of Wisconsin.
  • CPR/BLS (in accordance with American Heart Association) upon hire and appropriate renewal.
  • CEUs as required by state licensure regulations.
  • Ability to read and follow written instructions and document care given.
  • Demonstrated ability to supervise and direct Physical Therapist Assistants.
  • Self-directing with the ability to work with little direct supervision.

Job Duties

  • Respond to and initiate patient referrals.
  • Review available patient information and evaluate the impact of this information on the assessment and treatment process.
  • Develop and implement an individualized treatment plan based on patient needs and containing achievable functional goals.
  • Document the course of treatment in a clear, concise manner, including a discharge summary utilizing the prescribed format.
  • Instruct Assistants in treatment procedures to be utilized and provide supervision while such staff is working with patients.
