The home health aide is a paraprofessional member of the home care team who works under the supervision of a registered nurse or therapist and performs various personal care services as necessary to meet the patient’s needs.  The home health aide is responsible for observing patients, reporting these observations and documenting observations and care performed. The home health aide will be assigned in a manner that promotes quality, continuity and safety of a patient’s care.

Certification Requirements

  • Minimum 1 year of CNA experience
  • Certified Nursing Assistant Certificate
  • High School Diploma or GED
  • CPR/BLS (American Heart Association) upon hire and appropriate renewal
  • CEUs as required by state licensure regulations
  • Ability to read and follow written instructions and document care given
  • Self-directing with the ability to work with little direct supervision
  • Demonstrates tact, patience and good personal hygiene
  • Must be able to follow direction and accurately report to the Care Manager any changes in patient’s condition
  • Ability to work an 8 hour shift and to walk, bend, drive, stand, sit and reach constantly during that shift
  • Ability to push wheeled equipment throughout the facility
  • Ability to participate in sustained activity for up to 3 ½ hours at a time

Job Duties

  • Maintain patient comfort and safety.
  • Monitor vital signs; recording intake and output; reviewing patient chart; keeping family informed.
  • Follow all departmental policies and procedures as well as safety, environmental and infection control standards.
  • Assists with Activities of Daily Living (ADL) by serving meals and feeding patients as needed; offering fresh water and snacks; helping patients ambulate; and turning, positioning, and transferring patients.
  • Takes care of patients’ personal hygiene by assisting with or giving bed baths, baths, or showers; toileting by offering bedpans and walking patients to the toilet; providing backrubs and skin care; and keeping patients clean at all times.
  • Complies with HIPAA Privacy Act by protecting confidential patient information.
  • Follows all policies and procedures to maintain a safe home setting. Helps patients care for themselves by introducing and teaching assisted living techniques, such as using a cane or walker, using special utensils to eat, using special techniques and equipment for customer service needs.
  • Participates in all training and continuing education required. Maintains certification to meet federal, state, and accreditation standards.
  • Follows all departmental policies and procedures as well as safety, environmental, and infection control standards.
  • Records patient information by making entries in the patient’s chart; notifying Care Manager of changing or unusual conditions.
  • Helps family members care for the patient by introducing and teaching appropriate ways to lift, turn, and re-position the patient;
  • Adheres to PRN Home Health & Therapy’s policies and procedures, professional standards, federal, state, and local requirements.
  • Travel designated territory given by PRN Home Health & Therapy. Territories are subject to change based on staffing and business needs.
