About the position:
In this position as our SLP, you would work in both Home Health, Skilled Nursing Facilities and Outpatient clinics in Green Bay and the surrounding areas. You’ll evaluate and treat patients according to their physician by using therapeutic procedures designed to address deficits in speech, language, hearing, swallowing, etc. to improve their safety and quality of life. In this position you will travel to see patients in Green Bay, Marinette and locations in between. You’ll fit well in this position if you:

Part-Time Speech-Language Pathologist
Certification Requirements
- Certification Requirements
- Prefer a minimum of one year of clinical experience as a Speech-Language Pathologist.
- Graduate of an accredited Speech-Language Pathology program.
- Hold the Certificate of Clinical Competence in Speech/Language Pathology from The American Speech/Language/Hearing Association.
- Current Speech-Language Pathology license in the State of Wisconsin.
- CPR/BLS (in accordance with American Heart Association) upon hire and appropriate renewal.
- CEUs as required by state licensure regulations and American Speech Language Hearing Association.
- Ability to read and follow written instructions and document care given
- Self-directing with the ability to work with little direct supervision.
- Demonstrates tact, patience and good personal hygiene.
- Excellent oral and written communication skills.
- Must be able to follow direction and accurately report to the supervisory staff any changes in patient’s condition.
- This position requires frequent travel between patient homes; must be a license driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
Job Duties
- Respond to and initiate patient referrals.
- Review available patient information to conduct and document a thorough assessment of communication and cognitive function of the patient utilizing standardized procedures including home/vocational and equipment needs.
- Develop and implement an individualized treatment plan based on patient needs and containing achievable functional goals.
- Complete OASIS documentation if working in Home Health setting.
- Comply with evaluation, treatment and documentation guidelines.
- Record patient progress in the patient record a minimum of every thirty (30) days or as required.
- Revise treatment plans, as needed, throughout the treatment process.
- Documents the course of treatment from initial evaluation through discharge in a clear, concise manner in accordance with PRN Home Health & Therapy standards.
- Orient patients to program services and treatment procedures appropriate to their needs. Inform patients of any potential risk during any procedure.
- Work with clinical staff, patient and families to maximize patient’s use of functional abilities.
- Establish and teach home exercise programs for patients and/or care givers following discharge.
- Collaborate with clinical team to develop and educate patient and/or care givers on safe discharge plan
