Physical Therapist

About the Job: We’re looking for a Physical Therapist to see patients throughout Green Bay and the surrounding areas. This position will work in both outpatient and home health care settings (3 days a week outpatient and 2 days home health).

We’re Looking for a Physical Therapist (PT) Who: 

• Is willing to travel throughout Green Bay, WI and the surrounding areas to provide the best patient care around, primarily to patients in assisted living facilities and skilled nursing facilities!
• Loves working independently, but wants to have a team who is always there to support them when needed.
• Is a great communicator, both written and orally.

Certification Requirements

• Graduate of an accredited Physical Therapy curriculum.
• Current license/certification in Wisconsin.
• Current American Heart Association CPR Certification.

Job Duties

• Respond to and initiate patient referrals.
• Review available patient information and evaluate the impact of this information on the assessment and treatment process.
• Develop and implement an individualized treatment plan based on patient needs and containing achievable functional goals.
• Document the course of treatment in a clear, concise manner, including a discharge summary utilizing the prescribed format.
• Instruct Assistants in treatment procedures to be utilized and provide supervision while such staff is working with patients.
