Physical Therapy Assistant (PTA)
About the Job: We’re looking for a Part-Time or Per Diem Physical Therapy Assistant (PTA) to see patients throughout Fond Du Lac, WI. This position will primarily work in a skilled nursing facility setting. You will direct and aid patients in increasing gait, balance and functional independence through active and passive therapeutic exercise and muscle reeducation, gait and functional training and transfer activities, and more as delegated by the PT. The Physical Therapist Assistant participates as an active member of the interdisciplinary team.
If part-time we would require 3 days a week working 12-15 hours.

Certification Requirements
- Prefer a minimum of one year of clinical experience as a Physical Therapist Assistant.
- Graduate of an physical therapist assistant curriculum accredited by the American Physical Therapy Association
- Passed the National Physical Therapy Board exam
- Current Physical Therapist Assistant license in the State of Wisconsin
- CPR/BLS (in accordance with American Heart Association) upon hire and appropriate renewal
- CEUs as required by state licensure regulations
- Ability to read and follow written instructions and document care given
- Self-directing with the ability to work with little direct supervision
- Demonstrates tact, patience and good personal hygiene
- Excellent oral and written communication skills
- Must be able to follow direction and accurately report to the supervisory staff any changes in patient’s condition
- This position requires frequent travel between patient homes; must be a license driver with an automobile that is insured in accordance with state or organization requirements and is in good working order

- PHYSICAL DEMANDS & WORK ENVIRONMENT: Ability to move freely (standing, stooping, walking, bending, pushing, and pulling) and lift up to maximum of fifty (50) pound without assistance
Job Duties
- Provide Physical Therapy treatments to patients according to treatment plans developed by the Physical Therapist.
- Assist the Physical Therapist when performing tests, evaluations, and complex treatment procedures.
- Observe, record, and report to the Physical Therapist the patient conditions reactions and responses related to treatment.
- Maintain daily records of treatments and document progress notes on patients as directed by the Physical Therapist.
- Assist the Physical Therapist in consulting with family members, physicians and other health care professionals as appropriate.
- Assist with the care of the Physical Therapy department and equipment.
- Assist the Physical Therapist with in-service programs and other meetings as assigned by the Physical Therapist.
- Collaborate with the Interdisciplinary Team to identify and assist with obtaining patient specific DME including mobility aides, wheelchairs, cushions, orthotics, and adaptive equipment.
- Demonstrate competence and ability to use physical agent modalities including but not limited to heat, cold, electrical stimulation, ultrasound and manual therapy techniques.
- Access continuing education opportunities appropriate to discipline and responsibilities in order to maintain clinical competency.
- Collaborate with Physical Therapist and clinical team to develop and educate patient and/or care givers on safe discharge plan.
- Maintain current knowledge of community resources.
- Attend and participate in interdisciplinary team conferences, staff meetings, department/facility meetings and other meetings as required.
- This position must continually meet the productivity expectations set forth by PRN Home Health and Therapy. PRN has the right to adjust each individual employee’s productivity objective independently based upon said employee’s specific position responsibilities.
- Act in compliance with PRN Therapy Management Policies and Procedures, State, Federal and Professional Organization regulatory standards and guidelines.
- Participates in ongoing evaluation of the home environment for hazards or barriers to promote more independent living as delegated by the Physical Therapist.
- Maintain current state licensing, credentialing, and certification.
- Participate in quality improvement processes as directed by the organization.
- Complete all documentation, payroll and billing accurately and in a timely manner.
- This position may include working on weekends and holidays.
