The Registered Nurse Clinical Manager is responsible for the overseeing all the home health clinical services.  The Clinical Manager assumes responsibility for coordination of patient services, quality outcomes of clinical services, supervision and training of team members.

Certification Requirements

  • Registered nurse with current licensure to practice professional nursing in the State of Wisconsin.
  • A Bachelor’s degree in Nursing for an accredited program by the National League for Nursing.
  • Prefer a minimum of three years’ experience in a home health organization.
  • Current American Heart Association CPR/BLS for Healthcare Providers.
  • Promote and maintain a team-oriented environment.
  • Shows ability to organize and prioritize workload independently, multi-task and work in a fast-paced environment.
  • Ability to market and deal tactfully with customers and the community.
  • Has excellent attention to detail as well as, verbal and written communication skills.
  • Knowledge of home health regulations and accrediting agencies.
  • May require travel to satellite locations greater than 2 hours, as needed.
  • Must be license driver with an automobile that is insured in accordance with state and/or PRN’s requirements and is in good working order.
  • Excellent communication and organization skills.

Job Duties

  • Coordinates and oversees all direct and indirect patient services provided by nursing personnel.
  • Daily review and approval of clinical documentation in EMR.
  • Assures proper maintenance of clinical records in compliance with local, state and federal laws.
  • Oversight of compliance audits of all personnel documentation including, but not limited to: daily documentation, OASIS, coding, and plan of cares for accuracy and timeliness.
  • Monitor daily documentation alerts in EMR and rectify.
  • In conjunction with QAPI team, coordinate clinical data for QAPI indicators and present findings at QAPI meetings.
  • Field patient concerns/complaints, attempt to resolve or escalate to appropriate leadership staff, as needed.
  • In conjunction with leadership team, create, review and/or update policies and procedures.
  • Oversees and coordinates ancillary services.
  • Oversight of orders, face to face visits, and documents to ensure timely physician signatures.
  • Maintains communication with attending and referring Primary Care Providers for coordination of care.
  • Coordinates and participates in interdisciplinary meetings, as needed.
  • Provides patient care per Administrator.
  • Oversee Case Managers.
  • Oversight of nursing staff levels.
  • Collaborate with clinical.
  • Assist with oversight to Intake team.
  • Assist in onboarding and ongoing training to home health personnel.
  • Oversee competency assessments for all Home Health staff.
  • Works with the Therapy Manager and Administrator to develop strong clinical programming.
  • Develop and grow nursing programming.
  • Attend and participate in other department/facility meetings, staff meetings, or other meetings as required, Lead weekly interdisciplinary meeting.
  • Maintain current state licensing, credentialing, and certification. Participates in on-call duties as required, including weekdays, weekends and holidays.
  • May require working on holidays/weekends per rotation.
  • Supply management for patients and nursing personnel including ordering and maintaining an adequate supply level.
  • Maintain HR and clinical compliance, as required for nursing staff.
  • Providing performance reviews, assist with disciplinary action and develop performance improvement plans.
  • Will also complete all job duties of an RN.
