What services does a skilled nurse provide for home health?
Our nurses provide a number of skilled services including wound care, medication management and pain management.
What does a nursing visit typically look like?
When the nurse arrives they will typically take the patient’s vital signs and complete a quick assessment. All of the care they provide is under the patient’s care plan with their primary care physician (PCP.) It is the nurse’s responsibility to communicate any changes or updates to the PCP. The PCP and nurse will make recommendations if care needs to be changed or the care plan updated.
How long does a patient stay on nursing care?
It really varies for every patient. The goal of home health is for a patient to receive intermittent care for a period of time in order for them to meet their goals and continue as independently as possible. Our team will work with the patient and/or family member or friend to teach them how to properly provide care and manage their medical condition.
