Occupational Therapist (OTR)
PRN Home Health & Therapy is seeking a qualified Occupational Therapist to join our growing team in the Fond du Lac, WI area. This position would primarily work in the nursing home setting.

Certification Requirements
- Prefer a minimum of one year of clinical experience as an Occupational Therapist.
- Graduate of an accredited Occupational Therapy program.
- Passed the National Board of Certification in Occupational Therapy.
- Current Occupational Therapist license in the State of Wisconsin.
- CPR/BLS (in accordance with American Heart Association) upon hire and appropriate renewal.
- CEUs as required by state licensure regulations.
- Ability to read and follow written instructions and document care given.
- Demonstrated ability to supervise and direct Occupational Therapy Assistants.
Job Duties
- Respond to and initiate patient referrals.
- Review available patient information to conduct and document a thorough functional assessment of the patient utilizing standardized procedures.
- Develop and implement an individualized treatment plan based on patient needs and containing achievable functional goals.
- Comply with evaluation, treatment and documentation guidelines.
- Record patient progress in the patient record a minimum of every ten (10) visits or 30 days, whichever comes first.
- Revise treatment plans, as needed, throughout the treatment process.
- Documents the course of treatment from initial evaluation through discharge in a clear, concise manner in accordance with PRN Home Health & Therapy standards.
- Instruct Certified Occupational Therapy Assistants in treatment procedures to be utilized and provide supervision.
- Orient patients to program services and treatment procedures appropriate to their needs. Inform patients of any potential risk during any procedure.
- Work with clinical staff, patient and families to maximize patient’s ability to participate in activities of daily living.
- Collaborate with the Interdisciplinary Team to identify and assist with obtaining patient specific DME including wheelchairs, cushions and positioning aides, orthotics, and adaptive equipment.
- Demonstrate competence and ability to use physical agent modalities including but not limited to heat, cold, electrical stimulation, ultrasound and manual therapy techniques.
- Establish and teach home exercise programs for patients and/or care givers following discharge.
- Collaborate with clinical team to develop and educate patient and/or care givers on safe discharge plan.
- Maintain current knowledge of community resources.
- Attend and participate in interdisciplinary team conferences and other meetings as required.
- Attend and participate in other department/facility meetings, staff meetings, or other meetings as required.
- Access continuing education opportunities appropriate to discipline and responsibilities in order to maintain clinical competency.
- Assess Occupational Therapy Assistants’ competency on an annual basis per individual.
- Provide patient specific training to Occupational Therapy Assistants to enhance patient outcomes.
- Act in compliance with PRN Home Health and Therapy Management policies and procedures, State, Federal and Professional Organization regulatory and professional standards and guidelines.
- Promote the programs and services of PRN Home Health and Therapy through formal and informal interactions with the community.
- This position must continually meet the productivity expectations set forth by PRN Home Health and Therapy. PRN has the right to adjust each individual employee’s productivity objective independently based upon said employee’s specific position responsibilities.
- Maintain current state licensing, credentialing, and certification.
- Participate in ongoing evaluation of the home environment for hazards or barriers to promote more independent living.
- Participate in quality improvement processes as directed by the organization.
- Complete all documentation, payroll and billing accurately and in a timely manner.
- The duties of this position may include weekends and holidays.
- Perform other duties as assigned.
